Home-Delivered Meal Program (HDM)

Santa Clara County Senior Nutrition Program partners with Sourcewise and other organizations to provide home delivered meals to older adults. Contributions are suggested, but not required.

Who We Serve

Adults must be aged 60 or over and homebound due to illness, disability, or having difficulty obtaining food for themselves.

What We Offer

Trio Community Meals

Weekly delivery of 12 frozen meals, fresh salad, sandwich, and grocery items including whole grain bread, fruit, and milk.
Trio- HDM also provides Therapeutic Meals through 9/30/2024.
